How include Name in Birth Certificate in Bangalore

In this article, I will share my experiences to include a name in the Birth certificate. This is specific to Bangalore

My child was born in 2013, that time I have taken the birth certificate without name. Now I wanted include name in the existing birth certificate.

To do this activity first I contacted BBMP office. You need to identify a BBMP office where you will get a birth certificate. If you ask in the hospital (where the child was born), they will inform you about the respective BBMP office. BBMP office differs based on the location of the hospital.

You can visit BBMP office and go to births and deaths department. They will ask you to fill a form. When I went last time I brought the form and I attached for your reference. Along with application form, Following document xerox copies you need to submitFather/Mother ID proof Xerox copy

• Child birth certificate xerox copy which does not have name
• Father/Mother ID proof Xerox copy
• 20 rupees amount you need to give to get two copies

Once you give above documents, them they will give an acknowledgement slip. This you can furnish at the time of taking the birth certificate. While collecting father or mother need not go to collect the certificate. They may take 15 working days to produce this document

Other Information:

• If a child is below 6 Years age then you need not submit any document for child name proof.
• If a child is above 7 Years, then you need to submit a school certificate
• if a child is above 15 years then he need to furnish any educational document as a name proof
• Make sure you give proper surname for child with out any spelling mistakes
